Electronic forms

As a follow-up to a report by phone of an accident or a serious incident, at latest eight days after the accident/serious incident an electronic report shall be submitted to the AIB

Following report forms are available

Form 1: Accident/serious incident to aircraft/helicopters

Form 2: Accident/serious incident to gliders (in Danish only)

Form 3: Accident to microlight aircraft (in Danish only)

Form 4: Accident/serious incident to hot air balloons (in Danish only)

Form 5: Air Traffic Incident Report (ATIR)

Form 6: ATS reporting
